Really great work, this is the best version here in my opinion, and a tricky one to figure out for sure. Couple of comments:
- As has been mentioned, the G/F# should be a D/F# (200232) I think.
- I think the Csus2 sounds better as a Cmaj7 voiced as x32003, but that's definitely debatable.
- in the bridge, not sure what that Em/D is supposed to be (are you playing in drop D to get a bass in the D?), but I think a good ol Em7 sounds great there (020000)
- I think the C#/E in the bridge is better called a C#m7b5, but no biggie. But what's nice about calling it by its real name is you can then use the jazz voicings of that chord (which is a common jazz chord), like 9x998x, followed by a jazz voicing of the Cmaj7 (8x998x).
